CB Fleet Advisors uses information you submit through this website to respond to inquiries and handle requested DOT Health Assessments.
Information you provide
Our forms ask for your name, company and email address. You may include an inquiry topic and message. Assessment requests also ask for the fleet's USDOT number. Please do not submit account credentials, sensitive driver records or payment details.
How requests are handled
Conatus operates these forms for CB Fleet Advisors. Requests are saved in a restricted Google Workspace spreadsheet, and their details are emailed to CB Fleet Advisors. Notifications are sent through Conatus's Google Workspace account. Your email address is used as the reply address so CB can respond directly. Submitting a form does not enroll you in a newsletter, book an appointment or purchase a service.
Technical safeguards
We use a short-lived submission-protection cookie and browser session storage to help protect the forms and avoid duplicate requests. Our server uses a network address transiently for abuse protection; an opaque, daily keyed value is used for receiver rate limits. The inquiry spreadsheet does not store your raw network address. Google Workspace and our website hosting providers process information needed to provide these services and may maintain their own service logs.
Retention
We retain routine website inquiries for 24 months from submission, unless an ongoing customer relationship or a legal requirement calls for longer retention. Records no longer needed are removed from active records through manual review. Removal from active records may not immediately remove copies from provider backups.

Contact info@cbfleetadvisors.com to ask about information you submitted or request correction or deletion. CB and Conatus will review the request and any applicable retention requirements.
